What forms should be reviewed and filled out prior to meeting?
An Intake Form, Financial Policy and Patient Registration form should all be filled out prior to the first counseling session. These forms can be downloaded here in PDF format:
What insurances do you accept?
Each individual therapist at Counseling Alliance PLLC is responsible for his or her own credentialing with insurance panels. Please consult the Staff link to see which therapist(s) accept insurance. We are currently in the process of applying to several insurance panels so check back often if that is important to you. If you do not see your insurance provider listed here you may be able to be authorized through your insurance carrier for Out-of-Network services. Please contact your provider for details.
Do you provide medication management?
Not presently. At some point we plan to partner with a medical doctor/psychiatrist to provide that service. Currently we recommend you continue with your present physician.
Do you do counseling assessment?
Yes, several counseling assessments are offered. Often after the initial consultation, additional assessments will be recommended. The cost of those assessments depends upon which assessment is utilized.
Online payment option.
For your convenience, you can pay online. This is a secure payment through PayPal. When you get to the PayPal log-in page, you will need enter your session fee and log-in. If you do not have, or want, a PayPal account, you will see a link to pay by credit card as a PayPal guest.
